NON-MEDICAL TRANSPORTATION

Partnership provides the vehicle.
Transport to both medical and dental appointments.

Client must be able to get in and out of the vehicle independently or with the help of a care provider. Care provider may load and unload an assistive device (walker/wheelchair) in and out of the vehicle and may ride in vehicle with client. NO driver assistance with mobility and assistive devices.

Call at least 5 calendar days in advance to schedule. Have member ID, destination name, address, and phone ready. There is no guarantee of availability. If an appointment is urgent, you may call less than 5 days prior to appointment, but doctor verification of urgency is needed.

MEDICAL TRANSPORTATION

Transport to medical appointments only. NO dental.

Non-ambulatory client needing transport by wheelchair van or ambulance. Driver assists in and out of vehicle, but not in and out of the home. Care provider may ride in wheelchair van with client. Care provider may ride in ambulance with client depending on ambulance company policy and circumstances.

Physician prescription and prior approval required. Prescription may be for a single appointment or for multiple appointments with special circumstances (e.g. one month of physical therapy or one year of routine appointments with a single primary care provider). Call as soon as possible for reservation. There is no guarantee of availability. May accommodate urgent appointments if availability allows; then physician authorization must then be obtained within 15 days.

GAS MILEAGE REIMBURSEMENT PROGRAM

Call or go online to access a Reimbursement Trip Log and follow the instructions. Mileage reimbursement is available to family, friends, and care providers. Members cannot be reimbursed directly. Payment is $0.30 per mile. Call 10 days after submitting log to confirm receipt. Payment within 30 days.

TRANSPORT TO MEDICAL APPOINTMENTS

If you have mobility issues, call the service to clarify if the driver can help with ambulation, transferring, and/or assistive devices (wheelchairs/walkers).

AMERICAN CANCER SOCIETY ROAD TO RECOVERY PROGRAM

Volunteer drivers transport individuals to cancer-related medical appointments. Call as soon as possible, at least 5 business days in advance with pick-up and destination addresses. Local transport only. Geographic area covered depends on drivers available. Very limited availability. 1-800 number is available 24/7 and offers other cancer-related information. No cost.

ANGEL FLIGHT WEST

Non-emergency air travel for children and adults with serious medical conditions to medical appointments. Participants must be able to get in and out of a small aircraft with little to no assistance. No cost.

AREA 1 AGENCY ON AGING VOLUNTEER DRIVER PROGRAM

Transportation to medical appointments for individuals age 50+ and the disabled. Operates from Trinidad to Scotia and inland to Blue Lake. No cost.

BLUE LAKE RANCHERIA TRANSIT SYSTEM

BLUE LAKE BUS

Limited bus service weekdays between Blue Lake and Arcata. Stops at Mad River Hospital and UIHS. Free pass for Blue Lake residents who meet income requirement; contact St Joseph Blue Lake Community Resource Center: 668-5239; 111 Greenwood Rd Blue Lake.

BLUE LAKE DIAL-A-RIDE

Serves Blue Lake/Korbel/Glendale/Fieldbrook to McKinleyville/Arcata/Eureka. Operates Monday, Wednesday & Friday and the first Saturday of the month. Application requires healthcare provider verification of need. Cost $10-$12 per round trip.

DIAL-A-RIDE (managed by the Humboldt Transit Authority)

Transportation for seniors and disabled individuals unable to use public transportation. Application requires healthcare provider verification of need. Serves McKinleyville to Eureka. Cost $6 – $18 per round trip.

FERNDALE BUS (Ferndale Senior Resource Agency)

Ferndale residents age 62+ or disabled. Make reservation 24 hours in advance. Medical appointments and shopping. Rio Dell to McKinleyville. Rider must be able to access van independently. A care provider may ride along at no extra charge. No wheelchair lift, but may bring a wheelchair. Driver will help load/unload assistive devices. Price varies depending on location and time spent at destination.

FORTUNA SENIOR BUS

Transportation to medical and other appointments and shopping in Fortuna and once monthly trip to Eureka for shopping. Serves age 50+ and the disabled. Make reservations up to one month in advance for medical appointments and one week in advance for other appointments (e.g. beauty shop). For shopping, call by 8 am the day of needed ride. Weekdays only 8:30 am – 4 pm. Wheelchair lift. “Helpers” ride free. $2.50 one way or 15 rides for $22.50.

HUMBOLDT COUNTY MENTAL HEALTH

HUMBOLDT AREA FOUNDATION

ANGEL FUND

Travel grant up to $200 once per person per 365 day period to assist adults and children who have no choice but to leave the area for medical care.

OPEN DOOR CLINICS

Contact: Open Door patients contact your clinic

Limited funding for Open Door patients to assist with transportation to their Open Door and occasionally other medical appointments. Ask your clinic about bus passes or talk to your primary care provider for a referral to a case manager if you have an exceptional travel need. Gas vouchers may be available.
